I AM SECOND

Permission and Full Release

FOR GOOD AND VALUABLE CONSIDERATION, including (without limitation) the opportunity for me, or a minor for which I am the parent or legal guardian (the “minor”), to appear on screen and the website and/or social media of I Am Second, an initiative of e3 Partners Ministry (collectively referred to as “I Am Second”), receipt of which is hereby acknowledged, I hereby grant to I Am Second and to its affiliates, licensees, assignees, and successors in interest (collectively, “Licensees“), all rights of every kind and character whatsoever in perpetuity throughout the universe in and to the appearance of me, or the minor (whether occurring on one day or more than one), performance, voice, and the results and proceeds thereof, or any video or other media submitted to I Am Second (collectively, the “Appearance”), in connection with one or more audio/visual, print or other project, each of which may be shown commercially or to the public, including (without limitation) in connection with any video submitted to I Am Second (each a “Project” and, collectively, “Projects”).

I hereby authorize, in connection with one or more of the Projects in I Am Second’s sole discretion, the use of the name, likeness, biographical information, stories of me or the minor (whether public or private), and materials of every kind provided by me or my agents or representatives to I Am Second or its Licensees.

I hereby authorize I Am Second (including its agents, vendors and subcontractors) to photograph and record (on film, tape, digitally or otherwise), store or delete the Appearance and any Projects; to edit same at its sole discretion, or not to edit, in its sole discretion, and to include it with the performance and appearance of others, with other materials provided by me or my agents or representatives, with my stories, name, likeness and biographical information, and with sound effects, visual effects and/or music; to incorporate all or any portion into any Project or any other program or not; to use and to license others to use such recordings and photographs in any matter or media whatsoever (whether now known or hereafter developed or devised), including, without limitation, the unrestricted use for purposes of publicity, advertising, marketing, merchandising and sales promotion in connection with any one or more of the Projects. I further acknowledge that I Am Second exclusively owns all rights to the results and proceeds of the services and Appearance of me or the minor rendered in connection with the subject matter hereof as a work made for hire.

I hereby release I Am Second and the Licensees (including their respective agents, owners and representatives) from all claims of every kind and character related in any way to the subject matter of this Permission and Full Release, including but not limited to claims for libel, slander, invasion of the right to privacy or to publicity, any personal or business injury, or any other claim based on the use of the Appearance or any of the rights granted herein. My agreement, grant, release and consent to the terms of this Permission and Full Release are royalty free, worldwide, unconditional and irrevocable. No promises have been made to me to secure my signature to this Permission and Full Release unless otherwise noted herein.

To the extent I am agreeing to this Permission and Full Release on behalf of a minor, I swear, represent and warrant that I am the parent or legal guardian of the minor, with all legal rights to confirm the minor’s agreement as set forth in the Permission and Full Release. I agree and guarantee the above Permission and Full Release and freely allow the minor to appear in the Project and hereby grant to I Am Second and the Licensees all rights set forth above.

Upon the request of I Am Second, I agree to execute all documents and take any actions reasonably necessary to document or confirm the rights granted to I Am Second and the Licensees pursuant to this Permission and Full Release, including (without limitation) in connection with satisfying distributors or for chain of title or copyright conditions.
