Thank you for your interest in participating in an I Am Second focus group. The purpose of this focus group is to gather feedback, insights, and opinions regarding the Live Second Groups experience. By participating, you agree to the following terms and conditions.

1. Purpose of Participation

The purpose of this focus group is to help I Am Second better understand participant experiences, perceptions, and suggestions related to Live Second Groups. Information collected may be used to improve current programs, develop future offerings, and support organizational decision-making.

2. Voluntary Participation

Participation in this focus group is entirely voluntary. You may decline to answer any question or discontinue participation at any time without penalty or loss of benefits to which you may otherwise be entitled.

3. Eligibility

Participants must:

• Be at least 18 years of age, unless written parental or guardian consent is provided.
• Have participated in, attended, or have familiarity with Live Second Groups, as applicable.
• Agree to abide by these Terms & Conditions.

4. Session Recording

To accurately capture feedback, focus group sessions may be audio-recorded, video-recorded, photographed, or otherwise documented.

By participating, you grant I Am Second permission to:

• Record the session.
• Use recordings, transcripts, notes, and participant feedback for research, evaluation, training, and program development purposes.
• Store and review recordings internally and with authorized partners or contractors assisting with the project.

Participation is considered consent to such recording unless otherwise specified in writing.

5. Confidentiality

I Am Second will make reasonable efforts to protect participant privacy and maintain confidentiality of responses.

However, participants acknowledge that:

• Focus group settings involve multiple participants.
• I Am Second cannot guarantee that other participants will maintain confidentiality regarding information shared during the session.
• Participants are encouraged not to disclose sensitive personal, financial, medical, or confidential information during discussions.

6. Use of Feedback

All comments, suggestions, ideas, recommendations, and other feedback provided during the focus group become the property of I Am Second.

Participants understand and agree that:

• Feedback may be summarized, analyzed, quoted, or incorporated into future materials, programs, products, services, or communications.
• Individual names will not be used publicly without separate written permission unless otherwise agreed.

7. Compensation

Participants who complete the focus group may receive compensation, incentives, gift cards, stipends, or other benefits as communicated prior to participation.

Compensation details:

Amount/Type: $150

Participants who withdraw before completing the focus group may receive partial compensation at the discretion of I Am Second.

8. Code of Conduct

Participants agree to:

• Treat fellow participants and moderators with respect.
• Refrain from harassment, discrimination, or disruptive behavior.
• Allow others an opportunity to share their perspectives.
• Follow instructions provided by focus group facilitators.

I Am Second reserves the right to remove any participant whose conduct is disruptive, inappropriate, or harmful to the group environment.

9. No Guarantee of Program Changes

Participation does not guarantee that suggestions, recommendations, or feedback will be implemented by I Am Second.

10. Release of Claims

To the fullest extent permitted by law, participants release and hold harmless I Am Second, its affiliates, directors, officers, employees, volunteers, agents, and representatives from any claims arising out of participation in the focus group, except where prohibited by law.

11. Data Retention

Focus group recordings, transcripts, notes, survey responses, and related materials may be retained by I Am Second for research, evaluation, reporting, training, and organizational purposes in accordance with applicable policies and legal requirements.
